Patients like this one, who <strong>can be managed as outpatients<\/strong> should be started on <strong>empiric antibiotic therapy<\/strong> while awaiting culture results.\u00a0\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In children over the age of two months, UTIs are usually caused by gastrointestinal flora such as <strong><em>Escherichia\u00a0coli<\/em><\/strong>; therefore, the choice of antibiotic for empiric treatment should include coverage for this organism. <strong>Cephalosporins<\/strong>\u00a0are the preferred therapy when no allergy to or recent use of cephalosporins\u00a0exists. For patients like this one, who are being treated as outpatients, a first-generation cephalosporin like <strong>cephalexin\u00a0<\/strong>is\u00a0preferred. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Alternative agents available for patients over the age of two months when cephalosporins\u00a0cannot be used include <strong>nitrofurantoin,\u00a0amoxicillin-clavulanate,<\/strong>\u00a0and <strong>tremethoprim\/sulfamethosazole (TMP-SMX)<\/strong>. Choice of antibiotic should be tailored to the\u00a0<strong>results\u00a0of\u00a0the\u00a0urine\u00a0culture\u00a0<\/strong>when\u00a0it\u00a0becomes\u00a0available.
