points in our lives. Anxiety is a normal response to fear-provoking situations. However, for people who live with an anxiety disorder,&nbsp;<a href=\"https:\/\/www.nimh.nih.gov\/health\/topics\/anxiety-disorders\" target=\"_blank\" rel=\"noreferrer noopener\">these feelings are excessive, persistent, and disrupt the flow of daily activities<\/a>&nbsp;such as relationships, work, and school. The symptoms that accompany an anxiety disorder are frequently out of proportion to the trigger, or there&#8217;s no identifiable trigger present. It&#8217;s important to note that these anxiety symptoms should not be due to any underlying medical condition, substance use, or other mental health condition for a patient to be diagnosed with an anxiety disorder.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Having an understanding of the various\u00a0<a href=\"https:\/\/www.osmosis.org\/learn\/Anxiety_disorders:_Clinical_practice\" target=\"_blank\" rel=\"noreferrer noopener\">anxiety disorders<\/a>\u00a0is vital not only for mental health professionals but for\u00a0<em>all<\/em>\u00a0health professionals. Anxiety disorders are one of the most prevalent mental health conditions, with about\u00a0<a href=\"https:\/\/www.nami.org\/mhstats\" target=\"_blank\" rel=\"noreferrer noopener\">19.1% of U.S. adults<\/a>\u00a0being affected each year. Aside from how common these disorders are, some of the anxiety symptoms can overlap with other conditions, leading patients to a primary care physician, for example, rather than a psychologist or psychiatrist. They&#8217;re all different types of anxiety disorders that you should acquaint yourself with and be prepared to educate your patients about.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Generalized Anxiety Disorder<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Let&#8217;s kick off our discussion of anxiety disorders by learning about Generalized Anxiety Disorder.&nbsp;<a href=\"https:\/\/www.osmosis.org\/learn\/Generalized_anxiety_disorder\" target=\"_blank\" rel=\"noreferrer noopener\">GAD<\/a>&nbsp;is more common in women, and&nbsp;<a href=\"https:\/\/www.nimh.nih.gov\/health\/statistics\/generalized-anxiety-disorder#:~:text=Prevalence%20of%20Generalized%20Anxiety%20Disorder%20Among%20Adults,-Based%20on%20diagnostic&amp;text=An%20estimated%205.7%25%20of%20U.S.,some%20time%20in%20their%20lives.\" target=\"_blank\" rel=\"noreferrer noopener\">5.7% of adults<\/a>&nbsp;in the U.S. will experience it during their lifetime. In this condition,&nbsp;<a href=\"https:\/\/www.samhsa.gov\/mental-health\/anxiety-disorders\" target=\"_blank\" rel=\"noreferrer noopener\">patients feel intense and persistent anxiety<\/a>&nbsp;about many different aspects of their lives.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">GAD manifests with symptoms such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Difficulty concentrating<\/li>\n\n\n\n<li>Difficulty controlling worry<\/li>\n\n\n\n<li>Restlessness<\/li>\n\n\n\n<li>Fatigue<\/li>\n\n\n\n<li>Muscle tension<\/li>\n\n\n\n<li>Difficulty sleeping<\/li>\n\n\n\n<li>Headaches<\/li>\n\n\n\n<li>Stomachaches<\/li>\n\n\n\n<li>Nausea<\/li>\n\n\n\n<li>Diarrhea<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">To be diagnosed with&nbsp;<a href=\"https:\/\/www.osmosis.org\/learn\/Video_Case_Study_-_Caring_for_Patients_With_Generalized_Anxiety_Disorder\" target=\"_blank\" rel=\"noreferrer noopener\">Generalized Anxiety Disorder<\/a>, this excessive worry about elements of daily life should be present for six or more months, with the presence of three or more of the symptoms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So, how is GAD treated? It&#8217;s often with a combination of psychotherapy and medications. Often recommended for GAD,&nbsp;<a href=\"https:\/\/focus.psychiatryonline.org\/doi\/10.1176\/appi.focus.20200045\" target=\"_blank\" rel=\"noreferrer noopener\">Cognitive Behavioral Therapy<\/a>&nbsp;(CBT) is a form of psychotherapy that focuses on changing thinking patterns that are negative or unhelpful, introduces relaxation techniques, and reduces unproductive behaviors.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In terms of&nbsp;<a href=\"https:\/\/www.osmosis.org\/learn\/Antianxiety_Drug_Therapy\" target=\"_blank\" rel=\"noreferrer noopener\">medications<\/a>, antidepressants such as serotonin reuptake inhibitors (SSRIs) and serotonin and norepinephrine reuptake inhibitors (SNRIs) are helpful. Buspirone, which is an anti-anxiety medication, can also be used to augment the effects of antidepressants if necessary. Benzodiazepines can also be considered for use as long as it is only on a short-term basis&nbsp;<a href=\"https:\/\/adaa.org\/learn-from-us\/from-the-experts\/blog-posts\/consumer\/ssris-and-benzodiazepines-general-anxiety\" target=\"_blank\" rel=\"noreferrer noopener\">due to the risk of addiction<\/a>.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Panic_Disorder\"><\/span><strong>Panic Disorder<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Next, let&#8217;s learn more about&nbsp;<a href=\"https:\/\/www.osmosis.org\/learn\/Panic_disorder\" target=\"_blank\" rel=\"noreferrer noopener\">Panic Disorder<\/a>. Similar to Generalized Anxiety Disorder, this condition is more prevalent in women than in men. Overall,&nbsp;<a href=\"https:\/\/www.nimh.nih.gov\/health\/statistics\/panic-disorder\" target=\"_blank\" rel=\"noreferrer noopener\">4.7% of U.S. adults<\/a>&nbsp;will deal with panic disorder at some point in their lifetime. The main feature is&nbsp;<a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/anxiety\/symptoms-causes\/syc-20350961\" target=\"_blank\" rel=\"noreferrer noopener\">panic attacks<\/a>, which are episodes of profound terror, fear, or anxiety that tend to reach their peak after a few minutes. These panic attacks may occur several times a day or a few times a month.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other&nbsp;<a href=\"https:\/\/www.psychiatry.org\/patients-families\/anxiety-disorders\/what-are-anxiety-disorders\" target=\"_blank\" rel=\"noreferrer noopener\">key symptoms<\/a>&nbsp;include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sweating<\/li>\n\n\n\n<li>Feeling of heart racing<\/li>\n\n\n\n<li>Shortness of breath<\/li>\n\n\n\n<li>Chest pain<\/li>\n\n\n\n<li>Dizziness or lightheadedness\u00a0<\/li>\n\n\n\n<li>Hot flashes or chills<\/li>\n\n\n\n<li>Feelings of losing control<\/li>\n\n\n\n<li>Sensation of choking<\/li>\n\n\n\n<li>Afraid of losing control<\/li>\n\n\n\n<li>Fear of dying<\/li>\n\n\n\n<li>Nausea or stomach discomfort<\/li>\n\n\n\n<li>Shaking or trembling\u00a0<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Aside from these symptoms, important diagnostic criteria for&nbsp;<a href=\"https:\/\/www.youtube.com\/watch?v=YxELZyA2bJs&amp;ab_channel=OsmosisfromElsevier\" target=\"_blank\" rel=\"noreferrer noopener\">panic disorder<\/a>&nbsp;include recurrent panic attacks without a clearly identifiable trigger, as well as one or more months of persistent worry of having another panic attack or the consequences associated with them.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An ideal treatment strategy is a combination of psychotherapy and medication. CBT and SSRIs or SNRIs are generally the primary therapeutic approach.\u00a0<a href=\"https:\/\/www.osmosis.org\/learn\/Tricyclic_antidepressants\" target=\"_blank\" rel=\"noreferrer noopener\">Tricyclic antidepressants\u00a0<\/a>(TCAs) can also be used if the first-line medications are not effective. However, they should be used with caution due to their multitude of side effects. Hopefully, you confidently answered&nbsp;<a href=\"https:\/\/www.osmosis.org\/blog\/2023\/02\/01\/usmle-step-1-question-of-the-day-social-anxiety\" target=\"_blank\" rel=\"noreferrer noopener\">Social Anxiety Disorder<\/a>&nbsp;(SAD), previously referred to as social phobia. For people who have it, the anxiety associated with social situations is so great that it impairs people academically, occupationally, and socially.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.samhsa.gov\/mental-health\/anxiety-disorders\" target=\"_blank\" rel=\"noreferrer noopener\">Symptoms<\/a>&nbsp;to be on the lookout for are:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Difficulty making eye contact with others<\/li>\n\n\n\n<li>Anxiousness<\/li>\n\n\n\n<li>Stomach pains<\/li>\n\n\n\n<li>Feeling of heart racing<\/li>\n\n\n\n<li>Feelings of anxiety or embarrassment<\/li>\n\n\n\n<li>Difficulty being around new people<\/li>\n\n\n\n<li>Speaking with a very soft voice<\/li>\n\n\n\n<li>Stiff posture<\/li>\n\n\n\n<li>Fear of being judged by others<\/li>\n\n\n\n<li>Feeling self-conscious<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In terms of the diagnostic criteria, patients must present with a fear or anxiety that&#8217;s out of proportion to the situation, as well as avoidance of social settings and\/or instant fear once they&#8217;re in a social setting. As previously mentioned, their feelings should also impair functioning, with symptoms persisting for six months or more. Treatment options include CBT, SSRIs, or SNRIs, as well as benzodiazepines, as needed. For those with anxiety surrounding public speaking or performance anxiety, a&nbsp;<a href=\"https:\/\/www.osmosis.org\/learn\/Class_II_antiarrhythmics:_Beta_blockers\" target=\"_blank\" rel=\"noreferrer noopener\">beta blocker<\/a>&nbsp;can be prescribed to reduce shakiness and slow down the heart rate. However, be sure to watch for signs of emerging depression or anxiety, drowsiness, insomnia, hallucinations, or nightmares, which can be a side effect of use.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Specific_Phobias\"><\/span><strong>Specific Phobias<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Have you ever heard of arachnophobia, the intense fear of spiders, or claustrophobia, a fear of tight, closed-in spaces? These types of anxieties and fears are known as&nbsp;<a href=\"https:\/\/www.osmosis.org\/learn\/Phobias\" target=\"_blank\" rel=\"noreferrer noopener\">Specific Phobias<\/a>&nbsp;and involve the irrational fear of a situation or object. Patients aren&#8217;t immediately confronted with their actual phobia. Instead, the process may begin with looking at photos of the phobia or simply thinking about it before fully immersing themselves in the situation or interacting with the object. This condition frequently develops with Panic Disorder as well. In the most severe cases of Agoraphobia, patients&nbsp;<a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/anxiety\/symptoms-causes\/syc-20350961\" target=\"_blank\" rel=\"noreferrer noopener\">may not leave their homes at all<\/a>. Worldwide, there is a&nbsp;<a href=\"https:\/\/www.psychiatry.org\/patients-families\/anxiety-disorders\/what-are-anxiety-disorders\" target=\"_blank\" rel=\"noreferrer noopener\">prevalence of 1-1.7%<\/a>, including adolescents and adults.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&nbsp;In general, patients with Agoraphobia have a fear of two or more of these situations:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Lines or crowds<\/li>\n\n\n\n<li>Enclosed spaces<\/li>\n\n\n\n<li>Open spaces<\/li>\n\n\n\n<li>Public transportation<\/li>\n\n\n\n<li>Being outside of their homes alone<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The immense anxiety and avoidance of certain environments, along with social and occupational dysfunction, must be present for six months or more to make a diagnosis. Agoraphobia can be&nbsp;<a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/15769-agoraphobia#management-and-treatment\" target=\"_blank\" rel=\"noreferrer noopener\">managed<\/a>&nbsp;with CBT along with SSRIs or SNRIs.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Selective_Mutism\"><\/span><strong>Selective Mutism<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Have you ever met someone who refuses to speak in some environments but speaks freely in another? They may have Selective Mutism. Selective Mutism typically presents in childhood with a patient who has no issue using or comprehending language but, for one month or more,&nbsp;<a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/anxiety\/symptoms-causes\/syc-20350961\" target=\"_blank\" rel=\"noreferrer noopener\">fails to speak in those environments<\/a>, such as school. Patients may use gestures or other nonverbal forms of communication in environments where they fail to speak. As you can imagine, this condition can add to existing challenges with school, work, and social interactions. Selective Mutism typically&nbsp;<a href=\"https:\/\/www.psychiatry.org\/patients-families\/anxiety-disorders\/what-are-anxiety-disorders\" target=\"_blank\" rel=\"noreferrer noopener\">presents prior to age five<\/a>, with a&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10533577\/#:~:text=Selective%20mutism%20(SM)%20is%20an,and%20academic%20functioning%20in%20childhood.\" target=\"_blank\" rel=\"noreferrer noopener\">prevalence of 1-2%<\/a>, and may not be diagnosed until a child begins school.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/selective-mutism#management-and-treatment\" target=\"_blank\" rel=\"noreferrer noopener\">Treatment options<\/a>\u00a0for Mutism include CBT, family therapy, and SSRIs for anxiety symptoms.\u00a0<a href=\"https:\/\/www.verywellmind.com\/family-therapy-definition-types-techniques-and-efficacy-5190233\" target=\"_blank\" rel=\"noreferrer noopener\">Family therapy<\/a>\u00a0goals are to strengthen the relationships between family members, enhance communication, and address any underlying issues affecting the household.
