class=\"wp-block-paragraph\">You&#8217;ve conducted your research program and are excited by the findings. You&#8217;re\u00a0confident your paper makes a real contribution to your field, but how do you ensure\u00a0it&#8217;s\u00a0published in a reputable journal\u00a0with the broadest circulation?<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Why_conduct_research\"><\/span>Why conduct research?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">It begins, as everything worthwhile in science does, with research.\u00a0Research aims to address specific questions or problems, gather evidence, test hypotheses,\u00a0and draw meaningful conclusions. It serves to expand our knowledge base, inform decision-making, drive innovation, and\u00a0ultimately\u00a0deepen\u00a0our understanding of the world and its complexities.\u00a0Furthermore,\u00a0to practice evidence-based medicine,\u00a0we\u00a0need to\u00a0generate evidence, which\u00a0is done\u00a0through research. Evidence informs safe and effective patient care. Relying on common sense just isn&#8217;t enough when\u00a0it comes to\u00a0dealing with human lives, especially\u00a0in the realms of\u00a0designing clinical pathways and treatments.\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Similarly, to know where and how to get published, you need to conduct evidence-based research.&nbsp;There are around 30,000 biomedical journals, 6,000 of which lie within the field of medicine.&nbsp;Although this means you have a lot of potential publishing outlets for your work, it also means that these publications have&nbsp;very specific&nbsp;remits and editorial policies.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It&#8217;s\u00a0also true that rejection rates\u00a0remain\u00a0high, even for very accomplished\u00a0authors. For instance, <em><a href=\"https:\/\/www.thelancet.com\/\">The\u00a0Lancet<\/a><\/em>,\u00a0which ranks first among all general and internal medicine journals globally, accepts only 5% of the papers it receives. The problem is one of volume\u00a0and quality.\u00a0Each\u00a0month,\u00a0<em>The Lancet<\/em> receives hundreds of\u00a0submissions, and they only have room to publish a small fraction of these.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"The_Publication_Process\"><\/span>The Publication\u00a0Process\u00a0<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Given the many pitfalls of submission, it pays to understand the editorial process and the complex route to publication.&nbsp;Here&#8217;s&nbsp;an example of the submission and publication process at Elsevier:&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-embed is-type-rich is-provider-embed-handler wp-block-embed-embed-handler wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"Elsevier: Submission status explainer\" width=\"500\" height=\"281\" src=\"https:\/\/www.youtube.com\/embed\/agTOkZvIYNY?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Many papers are rejected by the chief editor even before they reach the peer review stage. This is often due to technical errors, poor English, incomprehensibility, or lack of a clearly defined purpose.\u202f&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Those that pass this&nbsp;initial&nbsp;screening are sent out for peer review, and then those reviews are assessed by the editor. At this stage, one of four things will happen:&nbsp;<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>The paper will be rejected.&nbsp;<\/li>\n\n\n\n<li>It will be sent back for additional peer review.&nbsp;<\/li>\n\n\n\n<li>The authors will be asked to make amendments or revisions and then resubmit.&nbsp;<\/li>\n\n\n\n<li>The manuscript will be accepted for publication.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Although the latter is the most desirable outcome, it&#8217;s more likely that revisions will be requested. The revision process can involve several iterations before a final draft is accepted.<br><br>Common problems and pitfalls which lead to rejection:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The paper doesn&#8217;t offer any new insights or proposals.&nbsp;<\/li>\n\n\n\n<li>The paper&#8217;s hypothesis hasn&#8217;t been made clear.&nbsp;<\/li>\n\n\n\n<li>The paper is over-ambitious in scope.&nbsp;<\/li>\n\n\n\n<li>The authors haven&#8217;t made their research process clear.&nbsp;<\/li>\n\n\n\n<li>The paper is poorly structured and hard to follow.&nbsp;<\/li>\n\n\n\n<li>The English used is unclear or ungrammatical.&nbsp;<\/li>\n\n\n\n<li>The sample size isn&#8217;t large enough to provide sufficient evidence.&nbsp;<\/li>\n\n\n\n<li>There are technical errors in the research methodology.&nbsp;<\/li>\n\n\n\n<li>The conclusions stated don&#8217;t follow the evidence presented.&nbsp;<\/li>\n\n\n\n<li>The language used is too emotional and leading rather than neutral.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">That last point is a subtle one.&nbsp;It&#8217;s&nbsp;easy to become caught up in enthusiasm when your research points to an extraordinary discovery. However, this emotion should not be present in your paper. Scientific papers present evidence and offer conclusions, but they leave any&nbsp;emotional response to the reader.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In other words, avoid terms like &#8220;extraordinary,&#8221; &#8220;interestingly,&#8221; or &#8220;strikingly.&#8221; By editorializing this way, you are telling the reader what to think rather than letting them reach their own conclusions.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"What_do_editors_look_for\"><\/span>What do editors look for?&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Now that&nbsp;we&#8217;ve&nbsp;listed how an editor might object to your paper,&nbsp;let&#8217;s&nbsp;take a more positive spin. What are they looking for? What would a dream manuscript&nbsp;contain, leading to an immediate acceptance (or a yes, following minor revisions)?&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An editor-in-chief scans through the titles, cover letters, and abstracts&nbsp;and then decides which papers to pass through to peer review. Which means it pays to spend time getting those three elements right.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At the peer review stage, successful papers will tend to:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Offer a clear and useful message&nbsp;<\/li>\n\n\n\n<li>Be delivered in a logical, standardized format&nbsp;<\/li>\n\n\n\n<li>Be easy to read.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">By standardized format, editors mean that it flows clearly, and includes the usual sections you&#8217;d expect from a scientific paper: an introduction, abstract, methods, results, discussion, limitations, conclusions, and so forth.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ease of reading is too easily overlooked, as writers can become lost in technical jargon and may forget that the publication they&#8217;re aiming at should have a general readership. In theory, any reader should be able to understand the paper, especially when we&#8217;re talking about health research.\u202f&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Remember that competition is high. In 2022, <a href=\"https:\/\/www.medline.com\/\">Medline<\/a>\u00a0added\u00a03,752 citation additions per day (three per minute)! If you want to stand out, ensure your work pleases an editor by fulfilling the following wish list:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Aims_and_Scope\"><\/span>Aims and Scope&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The paper sits well within the\u202faims and scope\u202fof the publication (more on that below).&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Impact\"><\/span>Impact&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The research offers a positive&nbsp;practical impact \u2013 it can make a difference in clinical outcomes.&nbsp;Although blue-sky research&nbsp;\u2013 which is&nbsp;conducted without a specific practical application or immediate goal in mind&nbsp;\u2013&nbsp;has a place in biomedical publications, health research should offer a practical contribution to clinical practice. In a word, your findings are\u202fuseful.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Research writer J.P. Your manuscript should have one clear idea, and each section should contribute to the exploration of that one idea.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One way to assess clarity is to have colleagues in a different discipline read your work. If they can understand it, readers of the large circulation journals will&nbsp;probably understand&nbsp;it too. If they&nbsp;can&#8217;t, pay attention to their feedback and make revisions for comprehension.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Purpose\"><\/span>Purpose&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Ideally, the authors clearly&nbsp;state&nbsp;the purpose behind the research and clarify why publishing their findings is important. If the research offers something new, then state that upfront.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Tone\"><\/span>Tone&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The writing is unbiased and neutral, allowing the reader to come to their own conclusions about the paper&#8217;s findings.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Validity\"><\/span>Validity&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The&nbsp;methodology&nbsp;is valid, the study design is good, and the statistics reported, along with their presentation, are sound.\u202f&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Completeness\"><\/span>Completeness&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Nothing is missing from the paper. For example,&nbsp;you&#8217;ve&nbsp;included a statement on the limitations of your findings, and all data is made available. All references are&nbsp;provided&nbsp;correctly.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Correctness\"><\/span>Correctness&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">There are no technical errors in the data presented. Tables and figures are complete and clear.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Language\"><\/span>Language&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This is related to clarity, but more specifically relates to the quality of the writing. The perfect paper reads easily without causing the reader to stumble over odd sentence construction or malapropisms.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If your first language is not English,&nbsp;it&#8217;s&nbsp;crucial to have a&nbsp;grammatically-adept&nbsp;native speaker proofread your paper before submission. Even when&nbsp;you&#8217;re&nbsp;adept with English, it pays to have others review your work for typos, grammatical errors, overlong sentences, and other stylistic issues.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Conclusiveness\"><\/span>Conclusiveness&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Simply put, the paper&#8217;s conclusions&nbsp;emerge&nbsp;naturally from the evidence presented.
