She <strong>does not want children <\/strong>and <strong>does not desire to become pregnant<\/strong>. She is interested in learning about sterilization. Her <strong>sex partners<\/strong> have all been <strong>biologically female<\/strong>, and she has had a <strong>single, long-term partner for the past year<\/strong>. Her medical history is notable for <strong>severe endometriosis, chronic pelvic pain, <\/strong>and an <strong>appendectomy for a ruptured appendix<\/strong>. She takes <strong>oral contraceptive pills (OCPs)<\/strong> for her endometriosis, which reduces pelvic pain somewhat. The physical exam reveals a <strong>4 cm well-healed incision<\/strong> in the <strong>right lower quadrant<\/strong>; on a <strong>bimanual pelvic exam<\/strong>, the patient experiences b, and the <strong>uterus has poor mobility<\/strong>. Comprehensive <strong>contraceptive counseling<\/strong> is provided. The patient is surprised to learn there are so many options and decides to research <strong>long-acting reversible contraceptive (LARC)<\/strong> methods. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At this time, why is permanent sterilization <em>contraindicated <\/em>in this patient?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>A. Uncertainty about permanent contraception<br><br>B. Having exclusively female sexual partners<br><br>C. Age &lt; 25 and lack of living children<br><br>D. Severe pelvic adhesive disease<br><br>E. Despite this, <strong>medical ethicists<\/strong> conclude that <strong>paternalism<\/strong> and <strong>infringing on one\u2019s reproductive autonomy<\/strong> are worse than allowing a patient to make a <strong>well-informed decision<\/strong> that they may regret later. Therefore, <strong>neither young age nor low parity<\/strong> are contraindications to the procedure in a <strong>well-counseled patient<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>D. Severe pelvic adhesive disease<br><\/strong><br><strong>Endometriosis<\/strong> and <strong>ruptured appendicitis<\/strong> can both lead to <strong>severe pelvic adhesive disease<\/strong>, which is also suggested by <strong>poor mobility of pelvic structures on exam<\/strong>. Pelvic adhesions do <strong>increase the risk of surgical complications<\/strong>. However, patients have the <strong>right to reproductive autonomy<\/strong>, and a <strong>well-informed patient<\/strong> may still choose <strong>permanent sterilization<\/strong>, even if she chooses to stay on <strong>OCPs<\/strong> to control <strong>pain or bleeding symptoms<\/strong> or is <strong>not a perfect surgical candidate<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>E. Sterilization is not contraindicated in this patient<br><\/strong><br>At the end of this encounter, the patient expresses <strong>uncertainty regarding her desire for permanent contraception<\/strong> by stating her <strong>intention to learn more about LARCs<\/strong>. Therefore, this patient should <strong>not undergo sterilization<\/strong> until she <strong>confidently rejects LARCs<\/strong> in favor of <strong>permanent contraception<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Main_Explanation\"><\/span>Main Explanation<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Sterilization<\/strong>, also known as <strong>permanent contraception<\/strong>, is a <strong>surgical procedure<\/strong> that is intended to <strong>permanently prevent future pregnancies<\/strong>. The <strong>only indication<\/strong> for permanent contraception is a <strong>patient\u2019s desire for the procedure<\/strong>. Likewise, the <strong>only absolute contraindication<\/strong> to sterilization is the <strong>lack of consent<\/strong> from a <strong>fully informed patient<\/strong>. This means that for the patient to provide consent, she must be <strong>free from any coercion<\/strong> and <strong>truly understand all the options<\/strong> and the <strong>implications of her decision<\/strong>. At the end of this encounter, the patient expresses <strong>uncertainty about permanent contraception<\/strong> by stating her <strong>desire to learn more about LARCs<\/strong>. Therefore, performing a sterilization procedure on this patient is <strong>contraindicated<\/strong> until she <strong>confidently rejects all other forms of contraception<\/strong> in favor of sterilization.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Although <strong>patient desire<\/strong> is the <strong>only indication for sterilization<\/strong>, numerous <strong>patient factors<\/strong> may be obtained in history and exams that make <strong>alternative forms of contraception<\/strong> more favorable, as detailed in the table below. The patient should be <strong>counseled on these alternatives<\/strong> to obtain <strong>informed consent<\/strong>. Additionally, the <strong>risk of unintended pregnancy<\/strong> should be weighed against the <strong>risks of surgery<\/strong>, and this should be <strong>discussed with the patient<\/strong> to obtain <strong>informed consent<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is important for the <strong>counseling provider<\/strong> to clearly explain all the <strong>relevant medical facts<\/strong> and to make a <strong>medically sound contraceptive recommendation<\/strong>. For example, the physician in the vignette should inform the patient of her <strong>increased risk of regret<\/strong> due to her <strong>young age<\/strong>, as well as the <strong>potential increased risk of surgical injury<\/strong> due to <strong>suspected adhesive disease<\/strong> from <strong>endometriosis<\/strong>, and that for these reasons, a <strong>LARC<\/strong> is a <strong>better option to consider right now<\/strong>. However, the <strong>final decision<\/strong> to proceed with sterilization should always <strong>lie with the well-informed patient<\/strong> and may be based on <strong>personal preferences<\/strong>.
