So, let&#8217;s review a few evidence-based treatments and <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/blog\/a-quick-mental-health-treatment-guide-for-clinicians-interventional-or-procedural-therapies\" rel=\"noreferrer noopener\">innovative mental health interventions<\/a>, so you&#8217;re up-to-date and empowered to offer your patients the most current, well-rounded care!<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Transcranial_Magnetic_Stimulation_TMS\"><\/span>Transcranial Magnetic Stimulation (TMS)<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Let&#8217;s start exploring new emerging therapies! <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6592198\/\" rel=\"noreferrer noopener\">Transcranial Magnetic Stimulation<\/a> is a noninvasive therapeutic procedure that generates a magnetic field through a coil placed on the scalp, stimulating certain mood-controlling areas of the brain to increase neurotransmitter release and improve impaired neuron function.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">TMS is generally used for treatment-resistant major depressive disorder. <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC2993526\/#sec1\" target=\"_blank\" rel=\"noreferrer noopener\">About 30% of patients don&#8217;t respond to first-line depression treatment<\/a>, like medication and psychotherapy. A TMS study found that <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10501497\/\" target=\"_blank\" rel=\"noreferrer noopener\">83% of patients who underwent TMS had a significant reduction in their depressive symptoms<\/a>, with <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10501497\/\" target=\"_blank\" rel=\"noreferrer noopener\">62% reporting continued symptom relief<\/a> after one year. In a separate study, researchers concluded that TMS <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6592198\/\" target=\"_blank\" rel=\"noreferrer noopener\">outperformed antidepressants in terms of cost-effectiveness<\/a> in patients with treatment-resistant depression. (Learn more about <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6592198\/\" target=\"_blank\" rel=\"noreferrer noopener\">TMS study outcomes<\/a>.)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Transcranial Magnetic Stimulation is also used to treat <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/notes\/Obsessive-compulsive_disorders\" rel=\"noreferrer noopener\">obsessive compulsive disorder<\/a> (OCD), with one study showing a <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC2993526\/#sec3\" rel=\"noreferrer noopener\">significant reduction in repetitive behaviors known as compulsive urges<\/a>, after TMS was administered. Yet another study found that <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6592198\/\" rel=\"noreferrer noopener\">38% of patients responded favorably<\/a> to TMS compared to the <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6592198\/\" rel=\"noreferrer noopener\">11% in a placebo group<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Although TMS is approved for the treatment of obsessive-compulsive disorder (OCD) and major depressive disorder (MDD), ongoing research is also exploring its potential in treating other psychiatric disorders such as <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/blog\/usmle-step-1-question-of-the-day-schizophrenia\" rel=\"noreferrer noopener\">schizophrenia<\/a>, <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/learn\/Tourette_syndrome\" rel=\"noreferrer noopener\">Tourette&#8217;s syndrome<\/a>, and <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/blog\/understanding-ptsd-a-helpful-guide-for-health-professionals\" rel=\"noreferrer noopener\">PTSD<\/a>. While the standard is <a target=\"_blank\" href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/transcranial-magnetic-stimulation\/about\/pac-20384625\" rel=\"noreferrer noopener\">five times a week for about four to six weeks<\/a>, <a target=\"_blank\" href=\"https:\/\/www.nature.com\/articles\/s41386-023-01599-z#Sec4\" rel=\"noreferrer noopener\">accelerated TMS<\/a>, which includes more sessions in a shorter time frame, is being investigated for its potential effectiveness. Researchers have linked depressive symptoms to dysfunctional NDMA receptors, low levels of glutamate, and brain-derived neurotrophic factor (BDNF). Ketamine <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10501497\/\" rel=\"noreferrer noopener\">blocks the NDMA receptors<\/a> in the brain while elevating the amount of glutamate, <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10501497\/\" rel=\"noreferrer noopener\">activating AMPA receptors<\/a> that allow neurons to communicate along different pathways. Researchers are still working to <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10501497\/\" rel=\"noreferrer noopener\">determine the exact way that ketamine infusions<\/a> work. However, they seem to deliver rapid antidepressant effects, providing patients with treatment-resistant depression as well as those with <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/blog\/understanding-mood-disorders-a-helpful-guide-for-health-professionals\" rel=\"noreferrer noopener\">bipolar disorder<\/a> and acute suicidality with another treatment option.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Along with rapid antidepressant effects, ketamine infusions also quickly reduce suicidal thoughts. <a target=\"_blank\" href=\"https:\/\/www.michiganmedicine.org\/health-lab\/ketamines-promise-severe-depression-grows-major-questions-remain\" rel=\"noreferrer noopener\">One study<\/a> found that about half of the participants with frequent suicidal thoughts had a huge drop in these impulses. Additionally, after just eleven days of ketamine infusions, <a target=\"_blank\" href=\"https:\/\/www.michiganmedicine.org\/health-lab\/ketamines-promise-severe-depression-grows-major-questions-remain\" rel=\"noreferrer noopener\">52% of participants<\/a> stated that their depressive symptoms reduced so much that they achieved remission. For the <a target=\"_blank\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33242561\/\" rel=\"noreferrer noopener\">40% of patients<\/a> with treatment-resistant major depressive disorder with cognitive deficits, ketamine infusions also positively affect their level of cognition by <a target=\"_blank\" href=\"https:\/\/www.health.harvard.edu\/blog\/ketamine-for-major-depression-new-tool-new-questions-2019052216673\" rel=\"noreferrer noopener\">improving memory and processing speed<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In addition to offering Ketamine infusions administered intravenously (IV), there&#8217;s also a nasal spray called <a target=\"_blank\" href=\"https:\/\/www.health.harvard.edu\/blog\/ketamine-for-major-depression-new-tool-new-questions-2019052216673\" rel=\"noreferrer noopener\">Esketamine (Spravato<\/a>) approved for use, which has been shown to <a target=\"_blank\" href=\"https:\/\/www.michiganmedicine.org\/health-lab\/ketamines-promise-severe-depression-grows-major-questions-remain\" rel=\"noreferrer noopener\">reduce depressive symptoms and suicidal thoughts<\/a>, all without the need for an IV. The challenge is that using a different type of drug delivery also means that the treatment interacts differently with brain receptors. Therefore, further research is needed to compare drug effectiveness and side effects for each type of administration.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Potential <a target=\"_blank\" href=\"https:\/\/www.health.harvard.edu\/blog\/ketamine-for-major-depression-new-tool-new-questions-2019052216673\" rel=\"noreferrer noopener\">side effects<\/a> of Ketamine infusions and nasal spray include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Nausea and vomiting<\/li>\n\n\n\n<li>Blurry vision<\/li>\n\n\n\n<li><a href=\"https:\/\/www.osmosis.org\/learn\/Hypertension:_Pathology_review\" target=\"_blank\" rel=\"noreferrer noopener\">High blood pressure<\/a><\/li>\n\n\n\n<li>Alterations in the perception of noises, time, textures, and colors<\/li>\n\n\n\n<li>Out-of-body experiences (<a href=\"https:\/\/www.osmosis.org\/notes\/Cognitive_and_dissociative_disorders\" target=\"_blank\" rel=\"noreferrer noopener\">dissociation<\/a>)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The <a target=\"_blank\" href=\"https:\/\/www.health.harvard.edu\/blog\/ketamine-for-major-depression-new-tool-new-questions-2019052216673\" rel=\"noreferrer noopener\">severity and timing of these side effects<\/a> may differ between the two drug administration methods, but more research is needed to confirm this.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While ketamine works quickly to improve depressive symptoms, it&#8217;s essential to keep in mind that it also has addictive properties, and patients with a history of <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/blog\/understanding-substance-use-disorder-a-helpful-guide-for-health-professionals\" rel=\"noreferrer noopener\">substance use disorder<\/a> should thoroughly discuss this treatment option with their provider before giving it a try.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There are also questions about <a target=\"_blank\" href=\"https:\/\/www.nimh.nih.gov\/news\/science-updates\/2023\/cracking-the-ketamine-code\" rel=\"noreferrer noopener\">how safe it is to use ketamine long-term, along with the cost-effectiveness of IV ketamine<\/a>. When administering ketamine infusions, patients should have their <a target=\"_blank\" href=\"https:\/\/www.apna.org\/ketamine-infusion-therapy\/\" rel=\"noreferrer noopener\">vital signs and level of consciousness monitored, along with checking for signs or symptoms of ketamine toxicity and dissociative effects<\/a>. Monitoring doesn&#8217;t stop in the treatment facility, so patients and caregivers should be <a target=\"_blank\" href=\"https:\/\/www.apna.org\/ketamine-infusion-therapy\/\" rel=\"noreferrer noopener\">well educated on potential safety concerns and establish an emergency response plan<\/a>. It&#8217;s important to consider these factors when deciding on a treatment plan for patients.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Psychedelic-Assisted_Therapy\"><\/span>Psychedelic-Assisted Therapy<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Another emerging therapeutic technique is <a target=\"_blank\" href=\"https:\/\/www.thelancet.com\/journals\/lanepe\/article\/PIIS2666-7762(23)00146-1\/fulltext\" rel=\"noreferrer noopener\">psychedelic-assisted psychotherapy<\/a>, which combines hallucinogenic psychedelic drugs and psychotherapy to address treatment-resistant depression and PTSD. MDMA (street name of Ecstasy or X) and psilocybin (mushrooms) are used in this treatment technique. <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10660711\/\" rel=\"noreferrer noopener\">MDMA increases serotonin, norepinephrine, and dopamine levels in the brain<\/a>, as well as decreases anxious and depressive feelings, which leads to <a target=\"_blank\" href=\"https:\/\/www.ptsd.va.gov\/professional\/treat\/txessentials\/psychedelics_assisted_therapy.asp\" rel=\"noreferrer noopener\">increased feelings of empathy, self-compassion, extraversion, and introspection<\/a>, along with altering patients&#8217; body image, sense of self, and perception. Overall, this type of therapy can help PTSD patients tackle their traumatic experiences with less fear and avoidance, allowing them to work through their experiences with a therapist.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Research on MDMA-assisted psychotherapy is ongoing, with one study noting that <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10660711\/#sec6-diseases-11-00159\" rel=\"noreferrer noopener\">76% of patients no longer met the criteria for PTSD<\/a> after this therapeutic technique, and with no side effects observed from the MDMA. Another study found that MDMA treatment in patients with severe PTSD was <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10660711\/#sec6-diseases-11-00159\" rel=\"noreferrer noopener\">well-tolerated and did not increase the risk of illegal substance use<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a target=\"_blank\" href=\"https:\/\/nida.nih.gov\/research-topics\/psilocybin-magic-mushrooms#effects-psilocybin\" rel=\"noreferrer noopener\">Psilocybin-assisted psychotherapy<\/a> is used for treatment-resistant depression. Psilocybin affects the serotonin 2A receptor, causing alterations in visual perception, improved mood, increased feelings of connection, and the ability for patients to recollect old memories. Patients who use psilocybin enter a dream-like euphoric state and may see or hear things that are not truly there. For some, it&#8217;s a blissful experience; for others, it&#8217;s frightening. Research is ongoing, with some studies finding that <a target=\"_blank\" href=\"https:\/\/www.nature.com\/articles\/s41598-024-53188-9\" rel=\"noreferrer noopener\">psilocybin quickly reduces depressive symptoms and improves mood long-term<\/a>, with some patients experiencing <a target=\"_blank\" href=\"https:\/\/www.thelancet.com\/journals\/lanepe\/article\/PIIS2666-7762(23)00146-1\/fulltext\" rel=\"noreferrer noopener\">sustained remission of depression after one treatment with psilocybin<\/a>. Much like standard exposure therapy, VRT works by placing patients in anxiety-provoking or stressful situations, but in the safety of virtual reality. Through VRT, patients can <a target=\"_blank\" href=\"https:\/\/www.medicalnewstoday.com\/articles\/vr-therapy#conditions-it-treats\" rel=\"noreferrer noopener\">practice skills, decrease fears, boost confidence, and work through a challenging situation they&#8217;ve faced in the past<\/a> while controlling the intensity of the exposure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While exposure therapy can be done without the use of virtual reality, VRT is an excellent option for treating a patient&#8217;s phobia of an object or situation that can be potentially dangerous. For example, an individual may have a phobia of a specific wild animal, and instead of exposing them to that animal in person, it can be done using VRT. VRT is also beneficial in treating PTSD by helping patients face traumatic memories or situations <a target=\"_blank\" href=\"https:\/\/www.medicalnewstoday.com\/articles\/vr-therapy#conditions-it-treats\" rel=\"noreferrer noopener\">in a controlled setting<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In terms of effectiveness, one study concluded that VRT <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6713125\/#s0004\" rel=\"noreferrer noopener\">reduces PTSD symptoms<\/a>, with the benefits continuing for about three months after treatment ended. VRT has also been shown to reduce symptoms of anxiety and depression by teaching skills like self-compassion and assistance with the visualization techniques used in cognitive behavioral therapy (CBT).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Other_Emerging_Therapies\"><\/span>Other Emerging Therapies<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Developed in the 1980s as a behavioral therapy integrating mindfulness and acceptance strategies with commitment and behavior-change techniques to increase psychological flexibility, <a target=\"_blank\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9702511\/\" rel=\"noreferrer noopener\">Acceptance and Commitment Therapy<\/a> (ACT) emphasizes accepting unfavorable experiences to help patients deal with their difficult emotions and commit to choosing actions that reflect their values. The goal of ACT is not to eliminate negative experiences, but instead to increase patients&#8217; involvement in activities reflective of their personal values, using <a target=\"_blank\" href=\"https:\/\/www.verywellmind.com\/acceptance-commitment-therapy-gad-1393175\" rel=\"noreferrer noopener\">six processes<\/a> aimed at empowering patients to make better choices despite difficult circumstances. An electroencephalography (EEG) monitors the patient&#8217;s brain waves, and, through this noninvasive feedback technique, patients learn to regulate their brain activity.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Neurofeedback therapy research can treat conditions such as <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/learn\/Attention_deficit_hyperactivity_disorder\" rel=\"noreferrer noopener\">attention deficit hyperactivity disorder <\/a>(ADHD), learning disorders, PTSD, substance use disorders, anxiety, and <a target=\"_blank\" href=\"https:\/\/www.osmosis.org\/video\/Trauma-_and_stress-related_disorders:_Pathology_review\" rel=\"noreferrer noopener\">trauma<\/a>. Typically, patients treated with CFT struggle to be kind to themselves, have strong feelings of shame, and are very self-critical. By using CFT and the skills cultivated throughout the sessions, patients become better at mood regulation and creating a sense of safety for themselves.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CFT is typically used to treat mood disorders, personality disorders, eating disorders, hoarding disorder, and psychosis. Research shows that it&#8217;s <a target=\"_blank\" href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0165032723000228#s0180\" rel=\"noreferrer noopener\">effective in improving self-compassion, self-reassurance, and decreasing depressive symptoms<\/a>.
