How will this affect residency applications and timelines? International medical graduates (IMGs) already have extra application steps and changes may add some new challenges. After passing the first two exams, STEP 1 and 2, you can practice medicine with supervision and apply to residency. STEP 3 may be completed before or during residency, and after passing you are officially allowed to practice medicine in the US without supervision!<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>STEP 1 and 2&nbsp;<\/em>may be taken in any order, but it is helpful to take STEP 1 first. Prior to testing, IMGs must meet USMLE STEP 1 requirements for international students by getting the Educational Commission for Foreign Medical Graduates (ECFMG) certification.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>STEP 1&nbsp;<\/strong>is an 8-hour, multiple choice exam covering basic medical sciences. The current passing score is 194\/300.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>STEP 2 CK<\/strong>&nbsp;(clinical knowledge) is a 9-hour, multiple choice exam focused on clinical knowledge. It emphasizes underlying disease mechanisms and how to diagnose and treat a patient. The current passing score is 209\/300.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Passing these two exams makes IMGs eligible for a J1 visa.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>STEP 3&nbsp;<\/strong>is a 2-day, multiple choice and computer-based case simulation exam covering application of both basic sciences and clinical knowledge. The current passing score is 198\/300.&nbsp;<em>STEP 3 is not required before residency<\/em>, but it makes you more competitive and eligible for the H1B visa. But, taking this exam before residency may delay your application.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"What_has_changed\"><\/span>What has changed?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>July 1, 2021:&nbsp;<\/strong><a href=\"https:\/\/docs.google.com\/document\/d\/1gL8qNnU8ZWFE5NzeCIWFuqgfVM5VHIdx9V5TXFNx2Is\/edit#heading=h.cmg4hfs29fx8\" target=\"_blank\" rel=\"noreferrer noopener\">USMLE<\/a>&nbsp;reduced the lifetime attempt limit policy from 6 to&nbsp;<em>4 attempts per STEP exam.<\/em><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"STEP_1\"><\/span><strong>STEP 1<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>October 2020:&nbsp;<\/strong><em>Increased the number of questions assessing communication skills<\/em>&nbsp;like patient empathy, privacy, consent, and reporting.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>January 26, 2022:&nbsp;<\/strong><em>STEP 1 will change to pass\/fail outcomes<\/em>, so you won\u2019t get a score.<a href=\"https:\/\/www.osmosis.org\/ultimate-guide\/usmle-step-1\" target=\"_blank\" rel=\"noreferrer noopener\"><\/a><\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"700\" height=\"250\" src=\"https:\/\/www.osmosis.org\/blog\/wp-content\/uploads\/sites\/2\/2024\/09\/usmle-step-1-everything-you-need-to-know-banner.png?w=700\" alt=\"\" class=\"wp-image-615\" srcset=\"https:\/\/www.osmosis.org\/blog\/wp-content\/uploads\/sites\/2\/2024\/09\/usmle-step-1-everything-you-need-to-know-banner.png 700w, https:\/\/www.osmosis.org\/blog\/wp-content\/uploads\/sites\/2\/2024\/09\/usmle-step-1-everything-you-need-to-know-banner.png?resize=300,107 300w\" sizes=\"auto, (max-width: 700px) 100vw, 700px\" \/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"STEP_2\"><\/span><strong>STEP 2&nbsp;<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>November 2020:&nbsp;<\/strong><em>Increased the number of questions assessing systems-based practice, patient safety, legal\/ethical issues, and professionalism.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>January 2021:&nbsp;<\/strong><em>STEP 2 CS (clinical skills) was permanently canceled and replaced by the OET for international students.&nbsp;<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>During the pandemic STEP 2 CS stopped for safety purposes since it required in-person student and patient actor interactions. Everyone did really well on it, so it didn\u2019t help differentiate between students. Also, it was only available in 5 cities.<\/li>\n\n\n\n<li>Instead, OET is a computer-based, international, English-proficiency exam that assesses healthcare professionals\u2019 language and communication skills.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"The_6_ECFMG_pathways_for_IMGs_applying_to_residency_in_the_US\"><\/span>The 6 ECFMG pathways for IMGs applying to residency in the US<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">When selecting your pathway, always go to the&nbsp;<em>first pathway that applies to you<\/em>&nbsp;(i.e. if pathway 1 and 3 apply to you, go with pathway 1).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Pathway 1:&nbsp;<\/strong>For people currently&nbsp;<em>licensed in another country<\/em>&nbsp;to practice medicine.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Pathway 2:&nbsp;<\/strong>For people who&nbsp;<em>passed a standardized clinical skills exam<\/em>&nbsp;for medical licensure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Pathways 3-5<\/strong>&nbsp;are similar and are for people whose&nbsp;<em>medical school is accredited<\/em>&nbsp;by one of these organizations or committees.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Pathway 3<\/strong>: accredited by an agency recognized by the World Federation for Medical Education\u00a0<em>(WFME)<\/em><\/li>\n\n\n\n<li><strong>Pathway 4<\/strong>: accredited by an agency received determination of comparability by the National Committee on Foreign Medical Education and Accreditation\u00a0<em>(NCFMEA)<\/em><\/li>\n\n\n\n<li><strong>Pathway 5<\/strong>: medical school issued the degree jointly with a US medical school accredited by the Liaison Committee on Medical Education\u00a0<em>(LCME)<\/em><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Pathway 6:<\/strong>&nbsp;For people who<em>&nbsp;don\u2019t meet requirements for pathway 1-5<\/em>&nbsp;or failed STEP 2 CS before it was canceled.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Upcoming_changes\"><\/span>Upcoming changes<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><em>Starting in 2024 only medical schools accredited by agencies recognized by WFME will be accepted.&nbsp;<\/em>So applicants from some medical schools may no longer be eligible for ECFMG certification!<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This map shows all countries currently recognized by the WFME in dark blue. The grey countries are not accepted and the blue are in the process of being accepted.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>But, don\u2019t panic!&nbsp;<\/em>This is only the&nbsp;<em>current status and more countries will be added<\/em>. Next year you will be able to go on their website and search your school to check if it will be accepted.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>If you start the ECFMG process (i.e. form 186) prior to 2024 these changes won\u2019t apply.<\/em><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"How_you_can_plan_ahead_for_success\"><\/span>How you can plan ahead for success<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"ECFMG_certification\"><\/span><strong>ECFMG certification<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Apply for the USMLE\/ECFMG identification number on the ECFMG website. They will email you a number to create an account.<\/li>\n\n\n\n<li>Apply for the certification by downloading and filling out form 186, paying the fee, and scheduling the notary with NotaryCam.<\/li>\n\n\n\n<li>Takes\u00a0<em>about 2 weeks.<\/em><\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"USMLE\"><\/span><strong>USMLE<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Apply for USMLE STEP 1 or 2 online and pay the fee<\/li>\n\n\n\n<li>Wait for ECFMG to verify your status by contacting your medical school online or complete form 183.<\/li>\n\n\n\n<li>Schedule the test on the Prometric website.<\/li>\n\n\n\n<li>Takes\u00a0<em>more than 1 month.<\/em><\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Restrictions\"><\/span><strong>Restrictions<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Check for the closest USMLE test center, there might not be one in your country. If you have to travel, try arriving at least one day in advance to make sure you know where you are taking the test and you are rested. Be sure to know what COVID-19 and travel visa restrictions might apply before traveling. However, taking it before will help your application, let you apply for your H1b visa, and you don\u2019t have to study for it during residency.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>What are the residency visas?&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>J1 is essentially an academic exchange visa<\/em>&nbsp;for international students to be in the US for school. It is sponsored by ECFMG for residency, and requires you to return to your home country afterwards for at least two years. There\u2019s a J1 waiver for doctors where you work one year in an area of the US lacking doctors &#8211; anywhere from New York to Alabama &#8211; instead of returning to your home country for two years.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>H1b visa is a dual intent visa<\/em>&nbsp;meaning that after school you might return to your home country or stay in the US. It is better for IMGs, but is trickier to get. As it requires passing all 3 STEP exams and is sponsored by the residency hospital.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Speak to your university about the proper forms for your situation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Should you take STEP 1 before it turns to pass\/fail?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>If you are ready to have a great score<\/em>, it would be an extra score to make you stand out on residency applications.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>BUT, don\u2019t take it if you aren\u2019t ready!&nbsp;<\/em>If you take it pass\/fail&nbsp;<em>you still have STEP 2 CK&nbsp;<\/em>to show off your clinical knowledge. Also, there are other important application sections to help you stand out like letters of recommendations, especially one\u2019s from a US doctor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>How do you stand out in electives or interviews?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>Get a great score on STEP 2 CK.&nbsp;<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Electives: Keep learning, stay engaged, and be interested.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Interviews: Be prepared, but don\u2019t forget to be yourself. If you are interested in a certain area it will show.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Are residency interviews going to stay virtual or return to in-person?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>We don\u2019t know yet.&nbsp;<\/em>It is good that they have been virtual because anyone can apply anywhere in the world without traveling.
