The patient is mechanically ventilated and sedated on a propofol and fentanyl infusion. During rounds, the patient appears increasingly cyanotic. The patient has a history of alcoholism and opiate use disorder and does not take any medication daily. Temperature is 37\u00b0C (98.6\u00b0F), heart rate 115\/min, blood pressure 136\/68 mmHg, respiratory rate 10\/min, and O2 saturation 93% on 100% FIO2. The patient is not over-breathing the ventilator. Bilateral breath sounds are present and cardiac examination is within normal limits. The patient has evidence of skin cyanosis. Stat blood gas results are obtained, which show pH 7.27 and a pCO2 50. The patient has evidence of <strong>cyanosis<\/strong> on examination with confirmed proper placement of the endotracheal tube, and ABG results demonstrate <strong>respiratory acidosis.<\/strong> Given this clinical setting, <strong>iatrogenic causes of respiratory acidosis secondary to ventilation should be considered<\/strong>, such as low tidal volumes, <strong>low respiratory rate<\/strong>, and equipment failure. A fault in any one of these settings may result in hypercapnia and subsequent acidosis. This patient\u2019s respiratory rate is most likely set too low given his RR of 10 breaths\/min and likely resultant respiratory acidosis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Many conditions fall can cause <strong>respiratory acidosis<\/strong>, including central nervous system (CNS), neuromuscular, obstructive, and parenchymal pulmonary etiologies; however, the presenting symptoms will be similar. Patients with respiratory acidosis may present with <strong>cyanosis, a decreased respiratory rate, or shortness of breath. <\/strong>Depending on the degree of hypercapnia, patients may also develop altered mental status and lethargy. Specific etiologies and appropriate treatments can be suggested by the clinical scenario\u2013e.g., ventilated patients may need ventilator settings adjusted.
