On <strong>magnetic resonance imaging (MRI)<\/strong>, <strong>longitudinal T2 hyperintensities<\/strong> in the <strong>posterior spinal cord<\/strong> may be seen, but these findings are <strong>neither sensitive nor specific<\/strong>. The <strong>most definitive evidence<\/strong> comes from <strong>positive syphilis serology<\/strong> and a <strong>reactive VDRL in the cerebrospinal fluid<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients may experience <strong>gait dysfunction<\/strong> because of <strong>motor weakness<\/strong>, <strong>sensory loss<\/strong>, <strong>poor coordination<\/strong>, or <strong>movement disorders<\/strong>. These issues can result in various <strong>gait patterns<\/strong>, including <strong>spastic<\/strong>, <strong>stomping<\/strong>, <strong>steppage<\/strong>, <strong>ataxic<\/strong>, and <strong>Parkinsonian gait<\/strong>. <strong>Assessing the pattern of gait dysfunction<\/strong> helps <strong>narrow the differential diagnosis<\/strong> and <strong>direct further diagnostic testing<\/strong>, such as <strong>imaging studies<\/strong>, <strong>genetic testing<\/strong>, <strong>laboratory tests<\/strong>, and <strong>electromyography and nerve conduction studies<\/strong>.
