A 45-year-old man comes to the office to be evaluated for right elbow pain for several weeks. The pain has limited his productivity at work as a plumber and worsens with extensor movements such as swinging his hammer. He has tried ibuprofen and over-the-counter pain medications with minimal pain relief. Past medical history is noncontributory. Vitals are within normal limits. On physical examination the patient has full range of motion of the elbow. There is tenderness to palpation at the lateral epicondyle, and pain is elicited at the right lateral elbow with resisted wrist extension, digit extension, and forearm supination. There is no swelling, warmth, or redness over the right elbow joint. Examination of the distal forearm, wrist, and hand is noncontributory. Which of the following best describes the etiology of this patient’s presentation?
