The patient was lowering himself from a pullup at the gym when he suddenly felt a “pop” followed by acute pain in the anterior left shoulder. The patient has a history of left supraspinatus tendinopathy that was treated with physical therapy. The patient is otherwise healthy and does not take any medications. Physical examination reveals ecchymosis, swelling, and tenderness over the anterior left deltoid. The patient's arm is pronated and flexed at the elbow to 90 degrees.
