A 44-year-old woman presents to the clinic after experiencing two episodes of “fainting” in the past month. She states that both episodes occurred while she was standing in line at the grocery store, when she suddenly started feeling warm, sweaty, and lightheaded just prior to losing consciousness. Each time, she regained consciousness within a minute and felt slightly nauseous, but not confused. Her past medical history is significant for depression and hypertension and her medications include sertraline and amlodipine. Temperature is 37°C (98.6°F), pulse is 77/min, respirations are 16/min, and blood pressure is 132/84 mmHg while supine and 124/82 mmHg while standing. Cardiovascular examination is normal, with no murmurs, rubs, or gallops. ECG shows normal sinus rhythm without ST or T wave abnormalities.
