A 61-year-old woman comes to the clinic because of a rash on her left breast. The patient first noticed a scaly, itchy rash that began on her left nipple two weeks ago and now has spread to the lateral aspect of the breast. Before the appearance of the rash, she experienced some burning pain on the left nipple. There is no significant family medical history. Vital signs are unremarkable. Physical exam is notable for an eczematous lesion involving the left nipple with several excoriations extending laterally. No nipple discharge is noted, and the lymph node basins are unremarkable. A full-thickness punch biopsy is taken, and several days later it shows malignant, intraepithelial adenocarcinoma cells (Paget cells) occurring singly or in small groups within the epidermis of the nipple. A diagnostic mammography shows a 3 cm lesion in the upper outer quadrant of the left breast, and a biopsy of the lesion shows ER+/PR+/HER2- invasive ductal carcinoma. Which of the following is the correct stage of this patient’s breast cancer?
