She has taken pyridostigmine daily for the past year and underwent a thymectomy six months ago. She started oral prednisone two weeks ago but has experienced hypertension, hyperglycemia, insomnia, and mood disturbance from the medication. She has a past medical history of gastroesophageal reflux disease and takes omeprazole. Temperature is 37.0°C (98.6°F), pulse is 68/min, respirations are 16/min, blood pressure is 136/86 mmHg, and oxygen saturation is 97% on room air. On examination, she has bilateral ptosis that worsens with sustained upward gaze.
