27-year-old man presents to the primary care physician for evaluation of right shoulder pain for the past several weeks. The patient describes the pain as dull and generalized and has noted that the pain worsens with overhead movements. The patient experienced a fall while rock climbing several weeks ago during which he landed directly onto his right shoulder. He immediately felt pain in the anterior shoulder. The patient has no significant past medical history and does not take any medications daily. Vital signs are within normal limits. Physical examination does not demonstrate obvious deformity, swelling, or warmth over the right shoulder. The patient has full range of motion of the right shoulder but notes worsening pain with abduction and elevation of the shoulderThe patient is asked to flex the shoulderextend at the elbow, and internally rotate with the arm until the thumb is pointing downward. When the examiner pushes the arm to the floor while the patient resists, there is deep shoulder pain. When the patient’s arm is supinated and the examiner pushes down, there is no pain. An X-ray of the right shoulder is shown below. Which of the following is the most likely diagnosis?
