A 41-year-old female comes to the physician for evaluation of shortness of breath over the past few months. She has had no fever or weight loss over this time period. Past medical history is noncontributory and she does not take any medications. The patient drinks 2 or 3 glasses of wine on the weekends and does not use tobacco or illicit drugs. Vital signs are within normal limits. On physical examination, the expiratory phase is prolonged and scattered wheezes are heard bilaterally. The liver is palpated 4 cm below the costal margin.
