A 79-year-old man is sent to the emergency department from an assisted facility for persistent fever for five days and new-onset disorientation. He has been in the facility for four months since having an ischemic cerebrovascular stroke. Medical history is significant for hypertension, diabetes, vascular dementia, and osteoarthritis. Current medications include insulin, amlodipine/valsartan, and donepezil. Temperature is 38.9°C (102°F), heart rate is 105 bpm, respirations are 26/min, blood pressure is 135/75 mmHg, and oxygen saturation is 96% on room air. He is oriented only to person.
