A 69-year-old man comes to the emergency department for evaluation of loss of sensation in the right upper and right lower extremity of one hour duration. The patient was working in his basement when he began losing feeling in his right arm and leg throughout the morning. He became more concerned when he developed a headache with associated non-bilious non-bloody vomiting. The patient has a history of untreated hypertension and hyperlipidemia. He does not take any medications. He drinks 6-8 beers on the weekends and has a 30-pack year smoking history. Current temperature is 37°C (98.6°F), pulse is 91/min, and blood pressure is 225/108 mmHg. Physical examination demonstrates an obese, ill appearing male with 2/5 weakness in the right upper and right lower extremities.
