An 82-year-old woman is admitted to the hospital for pyelonephritis and started on intravenous fluids, analgesics, and ceftriaxone. Over the next few days the patient develops worsening abdominal pain, distention, and nausea. The patient’s last bowel movement was prior to admission. Temperature is 37.2 °C (98.9 °F), pulse is 94/min, blood pressure is 111/78 mmHg, respirations are 16/min, and oxygen saturation is 99% on room air. On physical examination, the patient has a distended abdomen, mild diffuse tenderness to palpation and no guarding or rebound tenderness. A CT of the abdomen and pelvis demonstrates colonic dilatation without evidence of mechanical obstruction. Fluid hydration and electrolyte replacement are continued. A nasogastric tube is placed for bowel decompression and opioid pain medications are discontinued. Which of the following additional measures should be considered if the condition persists?
