A 9-year-old boy is brought to the pediatrics PA because of worsening abdominal pain over the past three months. He has not had a bowel movement in three days. The patient’s parent reports an 11-lb (5 kg) weight loss and fevers most nights over this same time period. Vital signs are within normal limits. Physical examination demonstrates abdominal distension and diffuse tenderness to palpation with a positive fluid wave test. A CT scan of the abdomen shows para-aortic and mesenteric lymph node enlargement and a 7cm x 7cm enhancing mesenteric mass. A biopsy of one of the aforementioned lymph nodes is performed and reveals a large number of medium-sized neoplastic B-cells with minimal cytoplasm and dead neoplastic cells, as demonstrated below.
