A 26-year-old man presents to the clinic with a one-week history of asymmetric joint pain that is most severe in the left knee and ankle as well as painful urination. He has no history of illness. He is sexually active with female partners and reports having unprotected intercourse recently. Temperature is 37.0°C (98.6°F), pulse is 82/min, respiratory rate is 16/min, and blood pressure is 124/76 mmHg. Physical examination reveals a warm, tender, swollen left knee and tenderness of the left ankle. Additionally, there is non-purulent conjunctivitis, white urethral discharge, and a rash on the palms and soles as seen in the image below. Laboratory studies are remarkable for an elevated ESR. An effusion is seen on X-ray of the knee. Rheumatoid factor is ordered, a genital swab is sent for nucleic acid amplification testing (NAAT), and arthrocentesis of the left knee is performed.
