A 28-year-old woman, gravida 1 para 0, at 20 weeks gestation is being evaluated for abnormal ultrasound results several days ago. A single amniotic sac was visualized and the amniotic fluid index during that time was 25 cm (normal 8-18). The pregnancy has been uncomplicated so far, and the patient has been consistent with prenatal care and taking prenatal vitamins. Medical history is notable for asthma, managed with albuterol as needed. Family history is unremarkable. Vitals are within normal limits. The abdomen is soft, and uterine size is large for gestational date. An amniocentesis is performed and reveals elevated levels of alpha-fetoprotein. Which of the following is the most likely diagnosis?
