A 50-year-old man presents to the emergency department because of an exquisitely painful rash on the right side of his chest wall. He states it started with a burning sensation in the same region over the past several days that mildly improved with acetaminophen, but then he began noticing the formation of “blisters” in the region. He states he had a fever and feelings of malaise several days ago. Past medical history is significant for chronic kidney disease secondary to diabetic nephropathy and renal transplantation two months ago. Current medications include insulin and cyclosporine.
