A 65-year-old woman is brought to the emergency department because of a two-day history of nausea, vomiting, and confusion. According to the patient's partner, she has lost 5 lbs (2.2 kg) in the past three months and often wakes up at night soaked in sweat. Medical history is unremarkable. The patient has a 50-pack-year smoking history. At the time of arrival, temperature is 37.0°C (98.6°F), pulse is 70/min, respirations are 16/min and blood pressure is 118/76 mmHg. Physical examination shows a somnolent female with moist mucous membranes and no jugular venous distention. Serum osmolality is 245 mOsm/kg and urine osmolality is 450 mOsm/kg.
