A 42-year-old man comes to the office because of watery diarrhea and right lower quadrant pain. The symptoms started 3 months ago and are not relieved by over-the-counter antidiarrheals. His temperature is 37.0°C (98.6°F), pulse is 80/min, respirations are 20/min, and blood pressure is 135/85 mmHg. Physical examination shows tenderness to deep palpation in the right lower quadrant. There is no rebound tenderness. Rovsing sign is negative. Colonoscopy reveals focal ulcerations adjacent to areas of normal appearing mucosa along with nodular mucosal changes in the colon. Biopsy findings of the involved tissue sample are shown.
