A 43-year-old woman presents to the emergency department for four days with nausea and diffuse abdominal pain. The patient noticed a bulge at her umbilicus after starting weightlifting six months ago. Over the past three days, the bulge has become red and painful to touch. There was initially sharp pain at the site of the bulge, but today the patient developed severe, diffuse abdominal pain. Temperature is 38.7 °C (101.6 °F), pulse is 102/min, respiratory rate is 18/min, and blood pressure is 138/79 mmHg. BMI is 39 kg/m2. On physical examination, the patient appears uncomfortable. There are hypoactive bowel sounds and diffuse abdominal tenderness to palpation without rebound or guarding. There is a protruding, severely tender, and erythematous umbilical mass.
