A 72-year-old man is brought to the emergency department by his partner for evaluation of confusion and frequent falls for the past week. He has been experiencing numbness in his hands and feet for the last month.  He had an episode of urinary incontinence this morning which is not usual for him. He adopted a vegan lifestyle one year ago. Past medical history is significant for type II diabetes and coronary artery disease for which he takes metformin and simvastatin.
