An 85-year-old man presents to a new primary care physician for a routine wellness examination. The patient states he has been well since his last visit several years ago, and he has no new medical issues. The patient has a history of hypertension, hyperlipidemia, coronary artery disease, and diabetes for which he takes amlodipine, carvedilol, aspirin, clopidogrel, and metformin daily.
