Introduction
To receive licensure in the US or apply for a medical residency program, you will need to sit for the USMLE® Step 1 exam, which tests your ability to apply knowledge, concepts, and principles, as well as evaluate your medical expertise. USMLE Step 1 Exam Dates
First, determine your availability. You’ll need to provide a three-month period of eligibility to take Step 1 through the ECFMG’s Interactive Web Applications (IWA). Each eligibility period will have a wide selection of test dates you can choose from.
Don’t wait to schedule a date, and make sure to plan ahead! The test slots fill up quickly, and as a result, not every date will be available. Make sure to apply as soon as possible to take Step 1 before the start of next year.
TIP: Your exam date cannot be scheduled on major holidays or during the first week of January.
Who is eligible to take the USMLE Step 1 Exam?
Before applying to take Step 1, you’ll need to find out you’re eligible. The following is what you need to be eligible for Step 1:
- Be enrolled in a medical school or a graduate school listed in the World Directory of Medical Schools that also meets the eligibility for ECFMG certification and examination (both at the time of your application and on the day of the test).
- Speak with an official at your school to get authorized using the certification of your enrollment status.
- Complete at least two years of medical school, which should encompass your curriculum’s basic medical science component.
In addition, you will need to fulfill any other mandated ECFMG criteria because you are from outside of the United States and applying to take Step 1 via the ECFMG.
Note: You must notify the organization that registered you for the exam if your eligibility status changes before taking the exam.

How to Apply for the USMLE Step 1 Exam through the ECFMG
Make sure to follow the necessary steps to apply for USMLE Step 1 through the Educational Commission for Foreign Medical Graduates (ECFMG) online portal.
Before applying, you should:
- Complete the Application for ECFMG Certification.
- Apply for and receive a USMLE/ECFMG identification number, as well as an online services password.
- Read the ECFMG Information Booklet and USMLE Bulletin of Information sections relevant to your chosen eligibility period. Also note that if your eligibility period extends into the next year, you should review information for that year as well.
- Read the application instructions.
Apply for Step 1 using the ECFMG’s Interactive Web Applications (IWA) page. Log into your account and complete the application, making sure to select a three-month eligibility period for the exam.
Once the ECFMG verifies your eligibility, they will issue a scheduling permit to you, and you can schedule your testing appointment.
TIP: Remember to select a date that falls within your previously chosen eligibility period, or you’ll have to reapply!
Costs and Fees Associated with the USMLE Step 1 Exam
Budgeting is essential to scheduling your board exams, as you must pay all applicable fees by the time you apply. Here are the costs you should be aware of.
- Before applying for Step 1, you must apply for ECFMG certification, which costs $160.
- The examination fee for Step 1 is $1020.
Make sure to factor in the International Test Delivery Surcharge, which can vary depending on where you’re testing.
How long will it take to get my USMLE Step 1 exam results?
USMLE Step 1 score dates are usually released about a month after you take the exam. There may be unexpected delays, so do your best to be patient and allow up to eight weeks to receive your results.
Tip: You will receive an email when your Step 1 exam score is available. Make sure to check it regularly!
