Multiple organ dysfunction syndrome or mods is the progressive dysfunction of two or more organ systems where homeostasis can't be maintained without intervention.
It occurs when an insult or injury to the body from conditions like sepsis, severe trauma or a critical illness initiates an uncontrolled inflammatory response depending on the severity and the number of organs involved mods can progress to organ failure and death.
As the nurse youll provide patient centered care for critically ill patients with mods. Now, mods can be primary or secondary, first primary mods results from a direct initial insult or injury to an organ that causes impaired perfusion and organ dysfunction.

This initiates an inflammatory response and the immune system including immune cells like neutrophils and macrophages becomes primed, meaning it's placed on high alert and prepared to respond.
If a secondary insult occurs when this happens, these primed immune cells release an outpouring of chemical mediators producing an exaggerated immune response called a cytokine storm.
This uncontrolled inflammatory response affects organs distant from the site of initial insult, resulting in secondary mods during secondary mods.
There's widespread endothelial cell damage. Extensive vasodilation occurs as endothelial cells release nitric oxide.
This along with massive capillary permeability results in interstitial edema, maldistribution of circulating volume and impaired tissue perfusion.
The endothelial cells also release pro thrombotic mediators which can lead to the development of excessive microvascular clot formation that obstructs blood flow and impairs perfusion even more together.
These events eventually result in profound tissue hypoxia. At the same time, the neuroendocrine system is activated causing an extensive stress response which involves the release of stress hormones that increase oxygen consumption and intensifies the metabolism of carbohydrates and fats to meet increased energy demands.
As metabolic demands increase with less oxygen available to fuel metabolic processes. Cellular dysfunction and acidosis eventually result.
Now, clinical manifestations of primary mods can be difficult to identify since they're typically not apparent until secondary mods occurs.

Where signs and symptoms vary based on organ involvement, moving from head to toe in the neurologic system manifestations can include fever and altered mental status such as confusion, agitation and decreased cognition.
In severe cases, seizures and coma can develop next in the pulmonary system. Tachypnea and dyspnea occur and may progress to respiratory failure and ARDS.
This manifests as noncardiogenic pulmonary edema presenting with crackles and diminished breath, sounds on auscultation as well as refractory hypoxemia, meaning blood oxygen levels remain low.
Despite supplemental oxygen administration, pulmonary hypertension may also develop in the cardiovascular system. Hypotension, tachycardia and a bounding pulse are evident.
And dysrhythmias may also occur, increased metabolic demands cause the heart to work harder which can lead to heart failure and eventually biventricular failure and cardiogenic shock may develop hematologic dysfunction also occurs including thrombocytopenia, anemia and clotting abnormalities that can progress to disseminated intravascular coagulation or dic.
Then in the gastrointestinal system, findings include diarrhea, constipation and vomiting. There may also be hemorrhage, abdominal pain and intolerance to enteral feedings.
Additionally, the gut barrier can be compromised, allowing bacteria in the gut to pass into the bloodstream, amplifying the inflammatory response damage to the liver can manifest as right upper quadrant pain, jaundice and increased liver enzymes also increased serum lactate can lead to acidosis and increased serum ammonia can lead to hepatic encephalopathy.
Lastly, in the renal system, acute renal injury can develop causing oliguria and elevation of nitrogen containing waste products called azotemia.
Laboratory test results typically show an elevated bun and creatinine electrolyte imbalances like hyperkalemia and metabolic acidosis.
Now, when caring for your critically ill patient with mods, your goals of care include promoting oxygenation and tissue perfusion.
Nursing Considerations4:41–7:24
Providing supportive care to preserve organ function and controlling and treating infection. Begin by assessing your patient's airway breathing and circulation or ABC S and placing them on a continuous monitor for any emergent life threating findings activate emergency protocols as needed support oxygenation with mechanical ventilation, positive end expiratory pressure or peep for short and prone positioning and collaborating with respiratory therapy.
You can also decrease oxygen demand through pharmacological sedation, temperature control and pain management. Monitor your patients hemodynamic status such as mean arterial pressure or map to promote tissue perfusion.
Administer and titrate the prescribed IV fluids and inotropic vasopressor and anti dysrhythmic medications to optimize cardiac contractility, support cardiac output and reduce myocardial workload.
Also administer the prescribed blood products to maintain normal hemoglobin and hematocrit levels to support oxygen delivery.
Frequently assess intake and output. Noting any fluctuations that may indicate renal hypoperfusion and examine their skin for signs of poor perfusion such as coolness, pallor or delayed capillary refill.
Administer the prescribed enteral feedings to meet nutritional needs and preserve and promote organ function. Monitor indicators of nutrition status such as weight and laboratory tests such as serum albumin if acute kidney injury is present, follow the prescribed regimen for hemodialysis or continuous renal replacement therapy, collect specimens for culture and sensitivity testing and administer antibiotics as prescribed, perform measures to prevent further infection, including hand hygiene and taking meticulous care of invasive devices like indwelling, urinary catheters and endotracheal tubes.
Also be sure to monitor critical laboratory tests including CBCC MP, liver function tests, coagulation studies, bun creatinine lactate and arterial blood gas results.
Finally work with the interdisciplinary team to address the initial cause of your patient's organ dysfunction and notify the healthcare provider if your patient's condition worsens or if you identify new signs and symptoms, also promote healing by providing a calm and comforting environment as well as emotional support.
